When should I see a Credentialled Diabetes Educator (CDE)?

* When you’re struggling to meet your health goals
* When there are changes to your medications or health conditions

A GP referral is not necessary; however, a referral may entitle you to a Medicare reimbursement.

We provide education and support for:
* Understanding diabetes
* Self blood glucose monitoring
* Continuous glucose monitoring
* Commencing new medications for diabetes (including insulin)
* Registration with the National Diabetes Service Scheme (NDSS)
* Food choices in type 2 diabetes
How much will it cost me?

A Diabetes Education session is $80.00

With a Chronic Condition GP Management Plan/Team Care Plan, Medicare reimburses $60.35 of the diabetes educator’s fee.

Diabetes Education session $80.00 - Medicare reimbursement $60.35 = $19.65 gap

A long consultation is $120.00

We can accept credit/debit card payments or cash. A tax receipt will be issued on payment of the fee.


Am I eligible for Medicare rebates?
If you have a chronic medical condition, such as diabetes, your GP may refer you for Diabetes Education, Podiatry or Dietician services under Medicare’s Chronic Condition GP Management Plan/Team Care Plan.

Am I eligible for a referral?
Speak to your GP, who will determine whether a GP Management Plan/Team Care Plan is appropriate for you and can refer you for a maximum of 5 allied health services per calendar year.
