What is a Home Medicines Review?

A home medicines review involves you, your doctor and a specially trained clinical pharmacist working together to help you manage your medicines. The pharmacist will visit you in your home and discuss your medicines with you. They will
answer any questions and make suggestions to you and your doctor to help you get the best from your medicines.

A Home Medicines Review does not cost you anything if you have a current Medicare or DVA card. You may need to pay to see the GP for the referral unless they bulk bill.
